Dental plans

- Helps pay for basic dental services needed by older adults.
- Apply at any time during the year.
- Easy method for payments.
- Extensive provider network.
- More than two annual cleanings are covered for members with specific at-risk conditions.

Plan overview
Both plans offer affordable monthly premiums. You're not paying for benefits* mostly used by younger adults!
Low plan
- Two cleanings/exams are covered anytime during the year.
- Pays 50% of basic in-network services.
- $1,000 annual maximum benefit.
High plan
In addition to low plan benefits:
- Pays 80% of basic in-network services.
- Up to $1,250 annual maximum benefit in some states.

Eligibility and enrollment
Eligibility
To be eligible for a vision or dental plan, you must:
- Be age 60 or older.
- Be an Everence Association member. If you are not already a member, you can download an application below:
- Live in a state where the plan is available. Currently, you can sign up for a plan in Arizona, California, Colorado, Delaware, Florida, Georgia, Idaho, Illinois, Indiana, Iowa, Kansas, Maryland, Michigan, Minnesota, Nebraska, Ohio, Oklahoma, Oregon, Pennsylvania, South Carolina, South Dakota, Tennessee, Texas, Virginia and West Virginia.
Enrollment
If you are eligible and are enrolling for the first time, you can sign up at any time.
- You enroll as an individual and have your own plan.
- We encourage you to enroll for one year. If you want to switch between the plan options, you can do so after you’re on your current plan for one year. If you cancel, you can re-enroll in one year after cancellation.
